New, Transferee, and Returnee Learners

Applicants must submit the following:

• Assessment Examination
• Two (2) colored photocopies of the PSA/NSO Birth Certificate
• Original Progress Report Card with Learner Reference Number (LRN)*
• Three (3) colored 2×2 ID photographs
• Certificate of Good Moral Character signed by the School Principal or Guidance Counselor*
• Medical Certificate stating that the learner is physically fit to attend school
• Certificate of Completion, Graduation Certificate, or Diploma (for incoming Grades 1, 7, and 11)

Old Learners (Previously Enrolled)

Applicants must submit the following:

• Original Progress Report Card (SF9)
• PECD Checklist (for Preschool to Grade 1 learners)

Additional Requirements

Preschool to Grade 1 Applicants:

• PECD Checklist

Grade 11 and 12 Applicants:

• ESC/QVR Certificate (if applicable)
• Brown Long Envelope (for SHS Voucher Program beneficiaries)

Note: Applicable only to learners with previous school records.

Foreign Pupils/Students

Foreign applicants must submit the following:

• Special Study Permit issued by the Bureau of Immigration
• Birth Certificate
• Medical Certificate
• Previous School Records (translated into English)
• Report Cards and/or Official Transcript of Records
